To register please call into the surgery to collect a registration pack or fill in the application forms online using the link below. Please note to be eligible for registration you must live within the Practice area (see below). Once completed please return the registration forms to us in person. We will require photo ID such as a driving licence or passport and proof of address such as a utility bill or bank statement. For children we will need to see their red book, birth certificate or passport. If you are prescribed medication by your previous GP and need to continue this, you may need an appointment for a medication review with our in-house pharmacist to continue to prescribe your medication. Please supply a list of your repeat medication and a member of the team will advise if you need an appointment once registration is complete.
